- *Once selected, live participants cannot be changed.
- *Please be sure to register the information of those who will be joining you.
- *If you enter the wrong phone number or Plus member ID, you will not be able to receive your tickets. Please verify your information correctly before applying.
<If your companion does not have a smartphone>
On the day of the performance, please present a valid form of identification at the designated window for identity verification. Both the ticket applicant and their companions should bring documents that allow for identity verification, and must arrive together with the ticket applicant. If identity verification cannot be completed, entry may be denied.
Please fill in the following information when applying.- (1) The Plus member ID of the accompanying person who is registered with the FC (if your ID begins with E, please enter it from E)

- (3) Input the phone number 【09099999999】 as the contact number for the companion's smartphone.
When entering companion information, please be sure to select 'I do not have a smartphone' for the question regarding 'possession of a smartphone device to display the electronic ticket.'
※ If you select 'I have a smartphone,' changes cannot be made after winning. If either the applicant or the companion does not have a smartphone, we cannot accommodate those without a smartphone. - Other details

<About valid identification documents>
- Both the applicant and accompanying person must bring one of the following valid identification documents.
Please bring one form of photo ID and two forms of non-photo ID.

If we are unable to verify your identity on the day of the performance, you may be denied entry. Please note that in this case, we will not be able to refund the ticket price or any fees incurred for purchasing the ticket.

- (3) My Number card *My Number notification card is not acceptable
- (4) Basic Resident Registration Card
- (5) Student ID card or student handbook with photo
- (6) Physical disability certificate
- (7) Mental Health and Welfare Handbook for Persons with Mental Disabilities
- (8) Rehabilitation record book
- (9) Residence Card
- (10) Special permanent resident certificate
- An official identification with a photo that verifies the following three points: name, current address, and date of birth, other than those mentioned above.
- ＜Caution＞
- 1.パスポート、学生証に現住所の記載がない場合は、その他に、氏名、現住所、生年月日の3点が確認できる公的な証明書(資格確認書など)をもう1点お持ちください。
- 2.身体障がい者手帳、精神障がい者保健福祉手帳、療育手帳について、顔写真が更新されていないものは本人確認ができないことがあります。その場合は、資格確認書など他の公的証明書をもう1点お持ちください。
- 3. If you are filling out the form to enter your current address by hand, please write your current address.

- ＜Caution＞
- 1. If you are filling out the form to write your current address by hand, please write your current address.
- 2. If the name and date of birth on the two documents you bring match, your current address only needs to be listed on one of them.
- 3. Documents issued for the purpose of being submitted to others, such as a resident's certificate, cannot be used as identification documents.
Additionally, cash cards, credit cards, membership cards, medical cards, and commuter passes are not considered official documents for identity verification.
